New Patient Information
What To Expect When You Visit Advance Rehabilitation
As our mission states, we strive to provide the highest quality rehabilitation services in the area.
In Preparation
Please dress comfortably for your appointments. You will be required to complete some new patient paperwork, which can be filled out just prior to your initial appointment. You should arrive approximately 15 minutes early in order to be ready for your appointment time with your therapist. Your new patient forms may also be printed from the forms section of this website. Insurance/Finances
On the first visit, you will receive a personalized insurance consultation session where all of your financial questions will be answered. By the close of this session, you should be comfortable with costs and available payment options offered by Advance Rehabilitation. It is our goal to meet your financial needs so that money will never interfere with your recovery.
First Clinical Visit
After your insurance consultation, you can also expect a thorough, interactive evaluation from a Licensed Therapist in order to establish a customized treatment plan that will assist you in meeting your functional goals. During this evaluation you will have the opportunity to discuss your condition, help set goals for your therapy, and gain a good understanding of what you may expect from attending physical therapy visits including prognosis, treatment interventions, and approximately how long it should take. At the end of the first treatment, you should receive a copy of your home exercises followed by a quick check out procedure. Expect approximately 1 ½ hour on the first visit.
Follow Up Clinical Visits
On all follow up visits, you simply sign in at the window in the lobby and expect to wait no longer than 5 minutes for a qualified aide to assist you to the treatment area where you will meet your therapist. From here, you will continue your treatment progression. Expect approximately 30 minutes to 1 hour per treatment.
Physician Communication
Communication with your referring physician is an integral part of your therapy to ensure consistent quality care as changes and progression occurs. You should expect a Physician Progress Note to be sent with you each time you return to your physician.
Follow Up and Customer Service
Customer service is an important part of our mission at Advance Rehabilitation. You should receive an anonymous patient survey for us to measure your satisfaction with our service. Please communicate with us on ways we can improve so that we may continue to provide the highest quality rehabilitation services possible! If you enjoyed your time at Advance Rehabilitation tell a friend. If you think we can improve - tell us.

Attendance and Compliance
It is important for you to realize that active participation in your health care is absolutely vital to a successful rehabilitation experience. One of the most important things you can do to speed up your recovery is attend your scheduled appointments. Your physician or therapist usually determines the number of visits that will be appropriate for your condition. Experience has taught us that the more consistent or compliant you are with your treatment regimen, the better chance you have of making a full recovery. We understand that situations will arise that make it difficult to keep all of your appointments. If you are unable to make an appointment, we ask that you give us a 24-hour notice of cancellation.
Dress Appropriately
You will need to wear loose fitting clothing that allows exposure of the injured body part. Shorts, T-shirts, Sweat pants and other "gym" clothing are all appropriate and preferred. Dressing rooms will be provided if needed.
Patient Financial Responsibility
Advance Rehabilitation understands that the cost of health care can be a burden. It is our intention to make it possible for all patients to receive the physical therapy they require, regardless of insurance coverage or financial circumstances. If you have healthcare insurance, we will contact your insurance carrier, verify your therapy benefits and discuss these benefits with you prior to your initial evaluation. Your estimated portion of the payment is due at the time of service, however, this is not always possible. Should you have any estimated financial responsibility, we will work with you to ensure that this will not stand in the way of your healthcare goals. If you do not have insurance benefits that cover physical therapy, we will work with you to ensure you receive the same standard of care as our patients with insurance coverage. By offering several payment options and arrangements, we can find a solution to any financial concerns.
